What we collect
When you take the tinnitus questionnaire, we collect your first name, email address, mobile phone number, your answers to the 25 questions, and your score. We also collect technical information about your visit: browser type, IP address, the ad or link that brought you here, and when you took the questionnaire. If you check the box to let us call or text you, we record when you agreed and the exact words you agreed to.
Why we collect it
We use your answers to calculate your score and show your results. We use your email to send you your score. We use your phone number only if you checked the box agreeing to calls or texts about tinnitus care. We use the technical information to measure our advertising and to spot junk or automated submissions.
Health information
Your answers describe how tinnitus affects you, so we treat them as consumer health data. We do not sell your information. We do not share it with advertisers, data brokers, or anyone else. We do not build advertising audiences from it. We do not send your answers, your score, your name, your email, or your phone number to Google or any other ad platform.
Cookies and measurement
We use a session cookie to run the site. We use Ahrefs Web Analytics to count page visits; it does not use cookies. We use Google Ads conversion tracking to count completed questionnaires. It is set up so Google receives no personalized-advertising signals and none of your answers or contact details.
How long we keep it
We keep your results so you can see them again. If you want your data deleted, email us from the address you used. We will delete every record tied to it within 30 days and tell you when it is done.
Your rights
You can ask us at any time to see, correct, or delete the information we hold about you. Residents of states with consumer health data laws, such as Washington, Nevada, and Connecticut, have these rights by law. We give them to everyone.
Security
The site runs over HTTPS. Your information is stored in an access-controlled database.
